Piles has been defined as vericosities of haemorrhoidal plexuses in the mucocutaneous lining of anal canal. It is common disease of Anal canal for which the patients complains to his physician.
Chronic constipation, irregular repeated stool, attack of diarrhoea & dysentery, pregnancy, hereditary factors,chronic straining at stool, abodminal tumour, cirrosis of liver and low fiberous dite.
Bleeding per rectum during defecation and protrusion of mass at anal orifice and replaced digitally or itself after defecation. Other symptoms of piles are mucous and blood discharge, itching at perianal region, anaemia due to chronic blood loss, palpitation, tachycardia and breathlessness.
Post operative severe bleeding, Anal abscess with fistula formation, recurrences and Anal Stricture.
After diagnosis and investigations the Kshar Sutra is ligated into the piles mass under local anaesthesia. All the piles masses are ligated at the same session. Patient is admitted in the hospital for one day and next day he is discharged. Post operatively 5-7 days Antibiotics and Analgesics are recommended. Hot sitz bath twice daily and mild lauxative like panchsakar chooran is given at night to pass out the soft and easy stool in the morning. 3 ml. Jatyadi tail enema is given during morning and evening daily before defecation.
Patient is asked to keep active himself and avoid long sitting, strenous works and two wheeler vehicle for one month till the wound is soundly healed up.
An average cutting time for Piles mass is 7 days and healing 15 days with a total recovery times of 21 days, when wound is soundly healed up and there is no pain, discharge or bleeding.
A Clinical study on 750 Patients of haemorrhoids done in 5 years at our center, treated by Kshar Sutra ligation showed 98.5% cure rate and 1.5 % patients complained bleeding per annum . Proctoscopy Revealed that there was recurrence of piles at new site rather then ligated one.
Thus Kshar Sutra treatment for piles is safe, easy and most successful with excellent result and negligible recurrences as compared to modern surgery where pain, bleeding, recurrences and anal Stricture are common post operative complications.
